Responsibility

  • help patients to adapt to and manage their physical and mental health long-term conditions, through the teaching of coping strategies
  • support the non-medical management of dementia and associated behavioural issues in care home patients
  • the aim of the HoH PCN Care Home Team is to improve and streamline the support provided by Primary Care
  • periodically review, evaluate and change rehabilitation programmes to rebuild lost skills and restore confidence
  • advise on environmental alterations, such as adjustments for wheelchair access, technological needs, and ergonomic support
  • yorkshire Health Network is a federation of all 17 GP practices within the Harrogate and Rural area of the Humber and North Yorkshire ICB
  • work with patients through a shared-decision making approach to plan realistic, outcomes-focused goals that are agreed and recorded
  • heart of Harrogate Primary Care Network is a four Practice network based across the centre of Harrogate Town with over 50,000 patients
