Responsibility

  • managing and prioritising your caseload, in accordance with the needs, priorities and any support required by individuals
  • accept referrals from GP practices and multi-disciplinary teams, local agencies and voluntary, community and social enterprise organisations
  • working as an integral part of the Social Prescribing Team to provide a service that empowers people to take control of their health and well-being and move the focus back onto what matters to them
  • work with your Social Prescribing Team Leader to undertake continual personal and professional development
  • you will work as a key part of the primary care network multi-disciplinary team, helping the team to strengthen community and personal resilience along with reducing health and well being inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing peoples active involvement with their local communities
  • adhere to organisational policies and procedures within the PCN, including confidentiality, safeguarding, lone working, information governance, and health and safety
  • co-produce a simple personalised care and support plan to improve health and well being, introducing or reconnecting people to community groups and statutory services
  • champion Social Prescribing and support educating non-clinical and clinical staff within their PCN multi-disciplinary teams on what other services are available within the community and how and when patients can access them
