Responsibility

  • work in partnership with the clinical auditor and healthcare assistant to achieve the chronic disease management of patients within the PCN, whilst providing supervision to the healthcare assistant
  • prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner
