we are a forward thinking and busy Practice, working across three sites
the role is varied and will require a high standard of IT skills and excellent organisational skills
previous experience would be an advantage
to be responsible for setting up new users on EMIS, the practice network and other systems
to ensure all Practice staff are adequately trained, providing an ongoing training programme to maintain individual competencies
to be an RA Manager responsible for requesting and managing Smartcards
routine maintenance of equipment

4 x GCSE Grade A to C or equivalent - E
level 3 Diploma preferably in IT / Business Studies or Health and Social Care or equivalent -D
experience of working in a public reception area - D
sound understanding of Primary Care information systems -D
experienced in working with EMIS Web, and/or other clinical systems and MS Office software - D
experience of working in a medical/clinical environment - D
ability to train staff - E
experience of working with a clinical computer system - D

GCSE or equivalent

Responsibility
respond to the daily healthcare needs of residents, ensuring timely and effective management of their medical conditions
ensure annual Health Checks are completed in conjunction with junior staff members
provide clinical expertise in managing patients with learning disabilities and complex physical health needs
act as an independent prescriber, ensuring safe and effective medication management
engage with families, carers, and other professionals to provide holistic and compassionate care
act as the clinical lead for patients with Learning Disabilities within the community, supported Living and across the 18 specialist learning disability homes, overseeing and coordinating care delivery
stay updated on best practices and contribute to service development to enhance care quality
conduct assessments, develop care plans, and review treatment strategies for residents in collaboration with the multidisciplinary team, the Learning Disability Liaison Team and the Community Learning Disability team
Requirements
registered as an Advanced Nurse Practitioner, or Advanced Clinical Practitioner with relevant professional body
extensive experience in caring for patients with learning and physical disabilities
independent prescribing qualification and experience
strong clinical decision-making and problem-solving skills
excellent communication skills, both verbal and written, with the ability to engage effectively with patients and colleagues
ability to work effectively as part of a multidisciplinary team
