Responsibility
- thrive Leaving Care is currently seeking a dedicated and compassionate Leaving Care Support Worker to join our team
- assist in developing personalised support plans for each individual, identifying their strengths, needs, and aspirations
- offer guidance and encouragement to develop essential life skills, such as budgeting, cooking, cleaning, and maintaining a tenancy
- collaborate with other professionals, including social workers, educators, and healthcare providers, to ensure holistic support for young people
- facilitate workshops and group activities to enhance personal development, resilience, and social skills
- this is an exciting opportunity to make a meaningful difference in the lives of young people who are transitioning from the care system to independent living
- advocate for the rights and needs of young people within the community and relevant agencies

Requirements
- A minimum of 1 year experience working with young people in a care or support setting
- knowledge and understanding of the challenges faced by young people leaving the care system
- strong interpersonal skills with the ability to build trusting relationships with young individuals
- excellent communication skills, both written and verbal
- empathy, patience, and the ability to remain calm in challenging situations
- ability to work independently as well as part of a team
- knowledge of local resources and services available to young people
