- hours available for weekdays, evenings and weekends

- job Main Purposes
- to provide community based activity opportunities for Adults with Learning Disabilities
- complete target setting and progressions evidence records, using ICT
- liaise with Area Managers to ensure social care is person-centred and assessed
- to facilitate the development of social interaction, friendship and community social inclusion for Adults with Learning Disabilities
- liaise with the individuals and their parents/guardians regarding targets, assessments and opportunities for social progressions
