Responsibility

- to identify current needs of the parent and other family members that impact on the young carers caring responsibilities and refer, as appropriate, to Adult or Children’s services
- to support other areas of Service Delivery as and when required in a proactive and positive way
- keep up-to-date records of contact with young carers and comply with SCC monitoring and information systems including database input of relevant information on individual young carers
- work alongside external agencies and schools to ensure multi-agency working to highlight and support the needs of young carers

- to attend meetings, conferences, and training relevant to the role as required and appropriate
