Requirements

  • registered nurse with NMC
  • full driving licence with use of own transport for business purposes
  • degree Level Nurse
  • experience of working in a community setting
  • delivering change and able to build strong relationships with internal and external colleagues and organisations
  • able to provide evidence of clinical knowledge
  • be able to provide details of employment history which details experience
  • flexible approach

Responsibility

  • the post holder will ensure adherence to Company Policies and Procedures at all times
  • to manage own workload generated from a defined population within the time available
  • discuss, highlight and work with team members to create opportunities to improve patient care
  • maintain appropriate patient and clinical records in line with Company Policies and Procedures and service specification
  • all staff are required to respect confidentiality of all matters relating to commercially sensitive information, patients and other staff members and must comply with the Company Policy and Data Protection Act
  • to work in partnership with primary, secondary, social, community and voluntary groups to reduce the need for acute, urgent and long term care by proactively identifying those at risk earlier to ensure the correct support is in place in a timely way
