Responsibility

  • sams Helping Hands is currently looking for an experienced Care Coordinator to join our growing team
  • you will be responsible for supporting the growth and development of the branch alongside the Care Manager and maintaining the weekly Care Assistant rotas
  • to carry out care tasks when required and to take part in the On-Call Service on a rota basis
  • as a Care Coordinator you will be liaising with both Social Services, Customers and their families. An understanding and knowledge of care is required for the role
  • we are looking for somebody who enjoys working in a fast paced environment who is keen to help us re-shape the face of adult social care

Requirements

  • experience in working in Domiciliary Care
  • ability to work to deadlines
  • produce carer rota's on a weekly basis
  • ability to resource and monitor care packages whilst matching care staff to service users in a person centred approach
  • to assist the Branch Manager in all aspects of care management in the designated areas
  • to take part in the On-call Service on a rotational basis
  • have a flexible working approach
  • excellent communication skills
