Responsibility
- the Mental Health Support Worker will support our service users in their complete involvement during their time at Root & Branch
- develop clear recovery plans in collaboration with service users through a variety of outcome measures, including ReQol questionnaire
- monitor and report recovery progress following regular review meetings with service users
- assisting in their travel to and from the service, their integration into the activities on offer and their journey towards achieving recovery goals
- this will involve both supporting them at the project as well as enabling them to organise their daily lives, resolve problems and conflicts and integrate into mainstream activities and employment or education
- provide support, information, advice and guidance or advocacy on a range of issues relevant to service users as appropriate
- as part of this person centred role you will ensure that Root and Branch services are benefiting individual's recovery goals, enabling our users by coaching and empowering and providing exit planning as an integral part of the process
- diffuse/resolve stressful situations with the use of effective communication skills, empathic responding and reflective practice
