Working hours

  • full-time

Responsibility

  • to ensure the delivery of outcome based care & support to all individuals, through assessment and person centre planning
  • to oversee the coordination of care and support staff , and ensure that a safe, consistent and reliable service is delivered
  • to monitor and oversee new individual referrals and client onboarding
  • working with the directors, to sustainably develop and grow the service
  • maintaining good communication with the employees, including staff meetings, office meetings, supervisions, communication apps, news letter etc
  • ensure that KPI's assessment and reports are met
  • participate in on-call duties as part of rota and assist with emergency cover and rota management issues as required
  • to develop and maintain positive lines of communication with all stakeholders including individuals, family members, care coordinator, commissioners and other external agencies

Requirements

  • care management: 1 year
