Responsibility

  • Q Care Assist is looking for a compassionate, detail-oriented, motivated, and hardworking individual to join our team as a Care Coordinator
  • the Care Coordinator will be responsible for providing the team with high quality detailed patient support services in the domiciliary sector
  • coordinate with the care team to ensure each patient receives the highest level of care and support
  • work closely with the Registered Manager and Care Coordinator Manager to ensure compliance with all departmental policies and procedures
  • write and review care plans
  • maintain communication with carers, patients, families and updating care plans and statuses, medication administration, other concerns, etc
  • respond promptly to any patient or family request for assistance in making decisions about care or accessing services

Requirements

  • providing care: 5 years
