Requirements
- must be licensed or eligible to be licensed in Nottingham. Qualified candidates must have a high school diploma or equivalent qualification

Responsibility
- develop and implement plans to engage families as well as plan and implement activities that promote wellness
- assist patients with self-administration of treatments, medications, and other needs
- provide care management services for patients in the clinic
- participate in the development of programs that will serve the needs of our patients and promote their wellness
- participate in the recruitment, training, scheduling, and evaluation of caregivers in our outpatient clinics
- as a Care Coordinator , you will be responsible for providing support to patients and their families through a variety of activities to promote wellness
- assist providers in our outpatient clinics with patient care activities
- you will work closely with the Medical Director and other team members to ensure that each family has a positive experience with our facility
