Responsibility
produce reports including monitoring reports as detailed in the service contract
encourage the team to contribute to the service planning, quality improvement and development of the advocacy project
ensure the team signpost and actively promote self-advocacy with service users where practicable
ensure the team capture high quality advocacy notes, outcomes information and other evidence within Charity Log, the case management system
to lead the provision of supporting older people with making decisions, ensuring they understand their rights, make effective choices and are able to voice their concerns around where they live, care planning, financial planning, benefits and hospital admissions
ensure updated information is disseminated amongst the advocacy team
to co-ordinate and manage the Advocacy Service, ensuring it is delivered on time, within budget and meets agreed key performance indicators detailed in the service contract
ensure quality standards are achieved by implementing operational policies and procedures to maintain professional and consistent advocacy services
