Responsibility

  • duties Maintain accurate records of care, consultation and medication administration, accurately monitoring and record observations as required
  • provide wound care Provide specialist clinics such as ear syringing, contraception etc
  • identify patient care planning needs and provide support and advice for long term conditions Implement Health promotion and well-being programmes Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice Develop and implement individual care plans for chronic disease patients, including providing LTC clinics and refer patients to secondary care as required Monitor patients' diet and exercise, including providing NHS Health Checks and reviews as required Working with the practice team to support the development, delivery and review of the surgery action plan including list growth, patient satisfaction levels, QOF, enhanced services, KPI targets, resource profile and cost management
  • collect specimens for pathology Provide travel advice and vaccinations Provide cervical cytology for the practice Process and interpret test results as required (e.g
  • cervical cytology results) Ensure data is coded appropriately Implement an effective immunisation programme in line with the national schedule Implement vaccination programmes for adults and children Support patients in the use of their prescribed medicines or over the counter medicines Liaise with external services and agencies to ensure vulnerable patients are supported Refer patients according to practice/local policies for substance abuse Support the clinical team with safeguarding matters in accordance with local and national policies Delegate clinical responsibilities as necessary and where appropriate Provide nursing expertise and guidance within the practice as necessary Act as the infection prevention and control lead for the practice, ensuring compliance with all relevant legislation and regulations, undertaking periodic audits as necessary Monitor and ensure the safe storage, stock rotation and disposal of emergency drugs and vaccines
  • manage stocks of supplies and provide cold chain management Ensure storage and disposal of medication is carried out according to policy Personal Qualities To adhere to the highest personal and professional standards as set out by the NMC and to put individual patient care as your priority
  • to ensure personal mandatory and organisationally required training is up to date At all times to behave as a professional clinician and act as ambassador for our practice You will be required to undertake all other duties that may be reasonably asked of you
