Responsibility
- to keep up to date with all communication/information systems
- to attend to care needs identified within the care plan ensuring respect, privacy and dignity at all times
- to support the preparation of review reports and attend reviews and other meetings as may be requested
- to maintain the safety of people at all times, alerting senior staff to any identified risks whether perceived or actual
- to undertake key-working responsibilities for individuals accessing the service
- to contribute to the assessment and evaluation of care plans, under the supervision of senior staff
- to record relevant clinical information in daily notes, produce reports and contribute to recording and monitoring of service user activity
- to support each Service User towards managing their own behavior by using the skills and approved approaches
