Responsibility

  • senior Care Coordinator you must be able to drive and have access to your own vehicle
  • to work along side the registered manager
  • oversee and manage the daily operations of the care coordination department
  • participate in the out of hours on call rota
  • perform regular Audit of across the service and complete actions
  • ensure the requirements of the CQC are met at all times
  • coordinate and schedule appointments for clients with healthcare providers
  • provide guidance and support to care coordination team members
