Responsibility
- senior Care Coordinator you must be able to drive and have access to your own vehicle
- to work along side the registered manager
- oversee and manage the daily operations of the care coordination department
- participate in the out of hours on call rota
- perform regular Audit of across the service and complete actions
- ensure the requirements of the CQC are met at all times
- coordinate and schedule appointments for clients with healthcare providers
- provide guidance and support to care coordination team members
