Responsibility
- contribute to specialised care plans, e.g. All care plans are person centered to support their individual needs
- support individual's end-of-life care following the specialised Care and Support Plan
- support all individuals with all aspects of daily living, e.g., personal care including dressing, undressing, washing, bathing, showering and continence needs
Show more +10 - support Individuals overcome any mobility problems and other physical disabilities, with the use of any aids, personal equipment within their Care and Support Plan
- support individuals to maintain a clean environment, e.g., making and changing beds, tidying rooms, doing light cleaning, laundry and removing rubbish and placing them in the appropriate outside bins
- support individuals with nutrition and hydration by, e.g., preparing meals, snacks and drinks. We offer extra support to individuals who require may require extra assistance to maintain their nutrition and hydration
- report immediately to the Registered manager any significant changes in the health or circumstances of an Individual
- encourage and enable Individuals to remain at home and as independent as possible with support
- support the Individuals to take their prescribed medication, including any PRN medication if required
- perform such other duties as may reasonably be required
- support any individual's mental and physical activity by, e.g., supporting social support to the individual, to maintain independence where appropriate, support individuals with there shopping needs, activities such as recreational activities, hobbies and interests
- ensure the health, safety, and welfare of the individuals you support and your colleagues
- always comply with company guidelines and policies, e.g., in respect of dress code, wearing of identity badges, travelling and security and safeguarding of the Individuals
