Responsibility
- we have an exciting opportunity for an enthusiastic Care Coordinator to join our team
- order drugs and consumables for the relevant team, ensuring stocks are managed and distributed to relevant sites in a timely manner
- provide a point of contact for the team for queries both clinical and administrative
- working with clinicians as part of a team a team to proactively identify and manage patients, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services
- undertake specific assigned tasks, project support or development work which may arise from time to time
- oversee the QoF, PCN DES and any other metrics for their relevant clinical area, ensuring that patients are requested to attend the practice appropriately thereby maximising patient care, clinical safety and income earning opportunities
- work with the scheduling team to ensure sufficient appointments are available and clinical staff scheduled accordingly to ensure the above objectives can be achieved
- manage any specialised equipment for the team, ensuring it is serviced at appropriate intervals and that faulty equipment is repaired or replaced
