Responsibility
working as part of the Prescription Team under the supervision of the practice pharmacist Assist with the management of prescription requests making medicine management changes at the point of ordering
these will normally be variably required medications such as laxatives/analgesia/ medication for self-care Refer to pharmacist or GP to review if they are regular, long term medication Supporting with general prescription queries Supporting with pharmacy stock shortages to facilitate provision of appropriate alternatives Managing FP10MDA prescriptions liaising with shared care partners to update existing prescriptions and set up new patients Working with the pharmacist, and being the first port of call for managing all prescription requests from home delivery companies Managing acute requests for appliances from other healthcare professionals, particularly dressing requests from district nurses and registered nurses in care homes Clinical audits and medicines management cost-saving initiatives Triage clinic letters and discharges; and, working within their scope of competence, ensure that they are updated or work-flowed to an appropriate clinician Supporting the Pharmacist to increase uptake of electronic Repeat Dispensing by targeting very specific patient groups reviewing their medication and, with support manage the transition and ongoing housekeeping when medications are stopped or changed etc
this includes: o Synchronisation: adjusting quantities on repeat to ensure all regular medications are prescribed for the same duration o Production of one-off prescriptions for prescribers to sign, in order to align medication o Deleting repeat items for duplicated medication o Generic to brand switches in accordance with CCG formulary approved items o Brand to generic switches o Problem linking of repeat medication o Monitoring for over-ordering o Review items that patients state that they are no longer taking and either: Remove from repeat
