Responsibility

  • � Manage any daily changes in rotas and provide to Community Carers
  • � To monitor and manage the Care Planner system, following up any tasks/alerts
  • � Supervision of staff including work-based assessments, reviews, and spot checks
  • � Preparation of Person-Centred Care Plans
  • � To ensure the shift is run safely and providing support to care staff on shift
  • � To act on any safeguarding concerns
  • � To undertake Personal Care or any task required by the service user when necessary to cover holidays and sickness within the team
  • � liaise with multi-professionals for the wellbeing of the service user
