Responsibility

- to assist with the delivery of the ‘Connecting Carers in the Community Project’ service in line with the National Lottery People and Places Funding Bid
- answer and managing calls and email inquiries into the South Wales fund from carers and social care teams and providers to provide them with information and support
- ensure NEWCIS is represented by supporting development of the integrated model of community services, providing good Carer support within the Community Support Services locality teams and the Community Resource Teams
- to identify carers who would like to explore opportunities that they feel will explore their wellbeing and ensure they are aware of the support available in the county
- to provide a specialised carer service for carers caring for a person with a long term condition or intending to care for a person with a long term condition
- facilitate BTG for carers across all the five South Wales areas, completing the paperwork and entering details onto Charity Log and appropriate spread sheets
- process referrals to the service which shall be from a variety of sources including the DCC, primary and secondary care including mental health teams, and self-referrals
- to work with carers to develop carer friendly initiatives, letting carers take the lead in their communities and develop projects that matter to them
- to explore and support Carers to facilitate groups that they would like
- to assist with the set up and delivery of the community outreach work and develop new carer respite break services in conjunction with the project manager and our partners
- encourage carers to make a positive impact by getting involved, sharing skills & strengthening resilience through co-production ensuring the priorities are in line with the outcomes required by the service specification
- to develop or be a part of in their communities
- to assist with the development of links with all partners listed in this proposal, to promote the project and make referrals and signpost service users as appropriate
- to ensure that barriers for those who find services difficult to access are actively minimised, providing a welcoming service
- recruitment and training/support of volunteer team
- to help people navigate and access appropriate services, ensuring that they are able to make contact with the right person for help
- recruiting, training and supporting carer and past carer volunteers in conjunction with the projects administrator
- carry out such other duties as may be required, which are consistent with the duties and responsibilities of the post
- provide one to one support, which may include an assessment of carer needs, to enable greater improvement in independence. This will include home visits
- provision of support to carers when their caring role ends and support their inclusion into society through accessing employment and community involvement
- to assist when necessary with the promotion of the service across NE Wales to carers, partners and referral agencies, through a variety of means such as leaflets, posters, newsletter and newspaper advertising
- undertake any other duties and responsibility commensurate with the post
- to undertake any other duties as reasonably requested by the Manager of NEWCIS and participate in NEWCIS activities and events throughout the year
- for Carers who wish to pursue employment, develop links with employers and support services e.g. Careers Wales/Job Centre Plus/ Private employers
- maintain all NEWCIS monitoring procedures and contribute to day to day running of the NEWCIS Wrexham office
- to identify and make links with other agencies and services operating in NE Wales, developing an accessible database which contains this information
- maintain and ensure confidentiality over personal information relating to individual users of NEWCIS and practice information
- ensuring post sorted each day and is given to appropriate staff members and outgoing post posted
- support the organisation of the newsletter, which includes the franking of the envelopes and arrangement of collection of the post bags
