- collaborate with the care management team to plan and assess individual care needs, creating personalized care plans
- monitor clients' health status and emotional well-being, promptly reporting any changes or concerns
- assist clients with various daily living tasks, including bathing, dressing, medication management, and meal preparation
- offer compassionate companionship and emotional support, fostering a positive and friendly environment

Requirements
- proven experience in domiciliary care, with a strong track record of delivering high-quality care services
- NVQ/QCF Level 3 in Health and Social Care or equivalent qualification is highly desirable
- demonstrated leadership abilities and the capability to mentor and motivate a care team effectively
- ability to conduct spot checks, competencies, and reviews for care staff
- possession of a valid driver's license is mandatory
- compassionate, empathetic, and patient approach towards providing care and support
- reliability,punctuality, and a strong sense of responsibility in meeting clients' needs
