- working with service users on opiate substitute therapy medications
- empower and motivate service users to identify and achieve desired outcomes
- assist in arranging appointments with relevant professionals

- we provide practical help and personalised support through our education, housing, justice and health and wellbeing services
- to contribute to meeting the overall SMART action plans of the Young Person
- work with people to help them articulate their goals, dreams and aspirations offering personal, beneficial and emotional support
- promote the rights, safety, and wellbeing of our young people
- working with service users who may be abstinent from alcohol or drinking at reduced levels
- develop a move-on plan with service users at the earliest opportunity, identifying realistic options for their future home
- provide advice and assistance on personal budgeting and claims for welfare benefits
- we work closely with people to help them build independence and to move forward to a better future
- to respond to challenging behaviours in a professional manner and calmly in a solution focused way
- regularly review the person centred support plan to update, record and monitor progress of each person
- create support, safeguarding and health and safety plans, keeping confidential records and write reports
- potential to facilitate some recovery focused group work
- ensure that, where applicable, housing and property management tasks are effectively undertaken in line with procedure and targets
- to support and encourage the young person to be a ‘good neighbour’ by promptly addressing any noise, visitor, ASB issues
