Responsibility

  • contact new Young Carers and welcome them to our service
  • gain an understanding of their caring role and develop individual support plans that reduce the impact of their caring responsibilities
  • make appropriate referrals for a statutory Young Carers Assessment
  • provide ongoing, time bound, one-to-one support, regularly reviewing their needs and adapting support plans as required
  • participate in multi-agency meetings such as team around the family, child protection conferences and child-in-need reviews
  • record and maintain accurate and confidential records
