care coordinator administrators play an important role within a PCN to proactively provide coordination of patient admin and navigation of care and support needs across health and care services of the Morley and District area
this is achieved by locating all the information about the patients identified care and support needs and by using their administrative skills to identify where the patient data should be input for data quality purposes
they will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing the PCN with high quality SystmOne based administration support
Requirements

social prescribing empowers people to take control of their health and wellbeing through referral to non-clinical social prescribing link workers
they give people time to focus on what matters to me and take a holistic approach to an individuals health and wellbeing.Take a whole population approach, working with a range of people who may benefit from social prescribing, including people who are lonely, have complex social needs, low level mental health needs and long-term conditions by helping people to identify issues that affect their health & wellbeing and co-produce a simple personalised care and support plan, and supporting people by connecting them to non-medical, community-based activities, groups and services that meet their practical, social, and emotional needs, including specialist advice services and arts and culture, physical activity, and nature and green based activities
by using coaching and motivational interviewing techniques to support people to take control of their own health and wellbeing as well as supporting development of accessible and sustainable community offers by working in partnership with VCSE organisations, local authorities, and others to identify gaps in provision, and take a community development approach to enabling growth in community activities and groups

job responsibilities The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide face to face structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, acute prescription request, while addressing both the public health and social care needs of patients in the GP practice that make up the PCN
the post holder will perform face to face medication review of patients with polypharmacy especially for older people, people in residential care homes and those with multiple co-morbidities and long term conditions
the post holder will provide leadership on quality improvement and clinical audit and well as managing some aspects of the Quality and Outcomes Framework
this role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within general practice

BTEC Level 3 Diploma in Pharmaceutical Science or NVQ Level 3 Diploma in Pharmacy Service Skills or equivalent with underpinning knowledge
current registration as a Pharmacy Technician with the General Pharmaceutical Council
minimum 2 years experience in a busy pharmacy setting
significant post-registration experience in any pharmacy sector e.g. hospital, community pharmacy or primary care
