- to manage a client caseload; deliver one-to-one support sessions and facilitate Groups
- based in Aberystwyth, working across Ceredigion
- to process referrals and conduct risk assessments; to co-produce support plans with service users; set goals; assess & manage needs and risks as well as measure outcomes
- to manage all administration processes accurately and efficiently, such as creating and updating records on our record information system\*, record appointments and up to date calendar. \*currently Lamplight
Requirements
- experience working with beneficiaries within the third sector
- level 3 in Health & Social Care OR Equivalent
- ideally, experience of managing small groups and empowering them
