Responsibility

  • deliver training to Locality Coaches, to ensure they are competent to deliver the service in line with the agreed program
  • maintaining training records and ensure Locality Coaches maintain their mandatory and operational training requirements
  • monitor performance by undertaking regular audits to ensure Locality Coaches adhere to service delivery standards
  • undertake monthly performance meetings to review performance against individual KPI targets
  • provide reports on the performance of individual Locality Coaches
  • work with the Support & Performance Manager to address issues of underperformance of individual Locality Coaches
  • support and engage with PCN’s, General Practice and primary care networks to ensure the agreed level of patient referrals
  • to ensure the appropriate level of referrals are made to our Digital Partner, Liva Healthcare

Requirements

  • bachelor’s degree or equivalent
  • at least three A-Levels or equivalent
  • health and Social Care level 5 diploma or equivalent
  • proven track record of delivering training to front-line workers
  • analysing and interpreting statistical performance information and acting upon it
  • experience of working within primary care or public health to deliver health and wellbeing services
  • excellent interpersonal, verbal and written communication skills
  • experience of communicating with individuals within senior roles, as well as those delivering the frontline service
