duty pharmacist to assess and signpost patients, including urgent on the day requests, and as appropriate provide definitive treatment and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team or externally; manage a non-filtered list and identify suspected red flags and underlying serious pathology and take appropriate action; use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice; actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person; complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles; build relationships and facilitate collaboration between practices and the wider system to support the delivery of care to patients; manage and co-ordinate the support of the PCN pharmacy team, including training needs; support the practices in achieving the Quality and Outcomes Framework targets, Network DES IIF targets and any targets from other organisations and; ensure good governance in place around prescribing and monitoring of medication lead, plan, develop and support the introduction of new working processes within the network
work with care home staff to improve safety of medicines ordering and administration
review the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines (i.e
attend and refer patients to multidisciplinary case conferences
