Responsibility
- clinical Pharmacist, Kingfisher PCN – Primary Care
- liaise with colleagues in community pharmacy to align support for medicines adherence
- job title Clinical Pharmacist
- prescribing advice to prescribers in practice e.g. temporary non-availability of drugs
- salary in line with indicative band 7 – band 8a
- ensure patient safety when they are transferred between care providers through reconciliation of prescribed medicines
- hours up to 37.5 hours/week – for individual negotiation
- lead for medicines optimisation scheme compliance, including care home support; discharge reviews; medication reviews; and QIPP
Requirements
- mandatory registration with General Pharmaceutical Council
- membership of the Royal Pharmaceutical Society
- undergraduate Pharmacy Degree
- completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council
- experience
- minimum of 2 years post graduate experience in pharmacy
- other post graduate studies such as diploma
- hold a prescribing qualification
Education
- participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development
- taking responsibility for own development, learning and performance and keeping up to date with relevant clinical practice and demonstrating skills and activities to others who are undertaking similar work
