Responsibility
- provide person-centered care and support to individuals in their own homes, care homes, or hospitals promoting their independence and well-being
- assist with daily activities, including personal care, medication administration, meal preparation, and household tasks
- foster positive relationships with service users, promoting dignity, respect, and inclusion at all times
- ensure accurate and timely documentation of care activities, including daily records, incident reports, and support plans
- collaborate with other healthcare professionals, families, and external agencies to ensure the best possible outcomes for service users
- respond to emergencies and crisis situations with empathy, composure, and appropriate interventions
- continuously monitor and evaluate the effectiveness of care plans, making adjustments as necessary to meet changing needs
- maintain confidentiality and adhere to all policies, procedures, and regulatory requirements
Education
- GCSE or equivalent
Requirements
- A caring and empathetic nature with a commitment to providing person-centered support
- excellent communication and interpersonal skills, enabling effective interaction with service users, colleagues, and other stakeholders
- ability to work independently and as part of a team, demonstrating reliability and flexibility in fulfilling shifts
- basic knowledge of health and safety practices in a care setting
- A valid DBS check and satisfactory references
- previous experience as a Support Worker or similar role is desirable but not essential. We welcome applications from individuals with a genuine passion for care and a willingness to learn
