Responsibility
- as a Mountbatten Carer you will be required to work unsocial hours as required, including weekends/nights and bank holidays
- work in collaboration with colleagues to ensure the delivery of high quality, safe and compassionate care and support
- to promote and maintain effective liaison with care team and other health and social agencies involved in the patients care

- you will work flexibly across all care services provided by Mountbatten which will involve working in patients own homes or other places of care

- to contribute to the assessment, implementation and monitoring of patients and carers, identifying physical and/or psychological problems and liaising with the appropriate health and social care team members
