Responsibility
- listen to children and young people’s views, record and act on these where appropriate
- provide regular individual support for your key child in a manner consistent with their wishes
- advocate for the needs and wishes of your key child
- to ensure the plans are accessible to all staff to ensure a consistent approach
- monitor and Evaluate Individual Support Plans to ensure they are still relevant and appropriate to meet your key child’s needs
- participate in, and complete reports for Statutory Reviews and meetings concerning your key child
- work in partnership with other agencies to the benefit of your key child
- compile Individual Support Plans in line with policy, ensuring your key child and significant others are fully consulted. Ensure their views and wishes are represented within these
