- the post holder will provide leadership on person centred medicines optimisation and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services through structured medication reviews
- patients who have received a comprehensive geriatric assessment as per the anticipatory care requirements
- the post holder will proactively manage: Patients in care homes as per the Enhanced Health in Care Home specification
- use appropriate clinical decision-making tools to support the delivery of SMRs and clearly record all SMRs within GPIT systems, as well as using appropriate clinical codes to signify the reasons for an SMR
- patients with severe frailty and patients prescribed high numbers of addictive pain management medication
- provide patient facing and remote clinics for those with questions, queries and concerns about their medicines in the practice
- prescribe medication to patients if requested where appropriate and to scope of prescribing qualification
- answers relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams and patients with queries about medicines
- manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging up those needing a review
- review the on-going need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines ensuring they get the best use of their medicines
