care coordinator

Responsibility

  • to allocate all client care calls to the carers, and ensure continuity at all times
  • your main responsibility is to match carers with customers by preparing advance rotas, and responding to any last-minute changes
  • you’ll brief carers prior to their placements, and manage relationships with clients, family members and healthcare professionals
  • to respond to short-term carer unavailability, and emergency cover requests
