Responsibility
to allocate all client care calls to the carers, and ensure continuity at all times
your main responsibility is to match carers with customers by preparing advance rotas, and responding to any last-minute changes
you’ll brief carers prior to their placements, and manage relationships with clients, family members and healthcare professionals
to respond to short-term carer unavailability, and emergency cover requests
to plan and manage carer holiday organisation and cover
this is a fast-paced role which involves a close working relationship with the branch manager, and the carers
to receive requests for domiciliary care referrals and liaise with the Registered Manager/Care Manager to organise care assessments

Requirements
must have a current British driving license and have own vehicle
have at least two years’ experience in a Care Coordinator role
