Responsibility
- communicating with management, care teams, the people we support and their families
- participating in Initials risk assessment, drafting care plans and attending reviews
- auditing records of care and support plans to ensure they are up to date and comprehensive
- overseeing staff rotas, on-call duties, emergency staff covers and handling annual leave requests
- closely Monitoring high risk clients to manage risks, liaising with care manager and clients’ families when required
- support clients with medication and medical equipment supplies
- contribute, staff Supervisions, team meetings and annual appraisals
Requirements
- care and Support Work: 1 year
