Responsibility
- to select and apply appropriate assessments for a designated caseload, addressing enabling the client in areas of self-maintenance, productivity and leisure
- to plan and implement individual and/or group interventions, in collaboration with the service users, to achieve therapy goals
- to provide support, advice and professional opinion to the wider multi-disciplinary team on matters within scope of practice
- to undertake interventions or sessions to provide a sustained programme of therapy or support
- to oversee a small team comprising of a Health and Wellbeing Coaches and social prescriber working together to support and activate patients presenting with mild or moderate mental health To set up processes to enable timely review and appropriate signposting to make best use of the available capacity of the team
- to update and undertake risk assessments and risk management plans
- to work with service users to identify goals as part of the overall care plan working within the care programme approach process
- to manage and prioritise a caseload
