Responsibility

  • clinical Lead
  • the clinical pharmacist will, as part of a multi-disciplinary team, be in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas
  • within this role they would be prescribers, or will be completing training to become prescribers, and will work alongside the general practice team
  • in addition, they would be expected to take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, thosewith multiple co-morbidities and people with other disabilities
  • this job description is intended as an outline of the general areas of activity and will be amended in the light of the changing needs of the service and organisation
  • job Types: Full-time, Permanent
  • taking a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists , liaison with community pharmacists and anticoagulation
