Responsibility
- our Inpatient Unit at Winsley is a safe and comfortable environment with dedicated medical and nursing care
- patients usually stay for a short time - between seven and ten days

- this is a band 5 position with potential development routes to band 6, where possible

- to respond to enquiries regarding the Hospice at Home Service during unsociable hours and allocate staff, as required
- the successful candidate will work alongside existing members of a very supportive Community Palliative Care Team and will contribute to the delivery of palliative care by providing expertise in decision making, advance care planning & holistic symptom control
- you will be expected to administer medication and treatment as prescribed in accordance with the NMC's and the Dorothy House policies and guidelines
- to recognise own support needs and those of colleagues and how they can be met
- to take charge, as required, and co-ordinate the nursing care, liaising with other members of the Multi-disciplinary Team
- during this role, you will work with the team using a patient-centred approach and evidenced based practice to assess, plan, implement and evaluate patient care and will co-ordinate care from admission to discharge
- to participate in education and training of staff in the clinical setting and more formal teaching programmes
- to supervise other members of the Nursing Team, such as Health Care Assistants and Volunteer Nurses, as required
- to assess the needs of patients and devise a plan of nursing care based on assessment of physical, emotional, spiritual and social needs, involving patients and carers in the process
- REGISTERED NURSE | Band 5 £14.53 - £17.69 per hour + Bank Enhancement
- to implement the planned programme of nursing care and where appropriate liaise with other members of the Caring Team who may be responsible for implementing specific aspects of nursing care

- to ensure and co-ordinate safe discharge of patients with an understanding of the needs of patients and carers in the community and the available services
- to record and report any accidents, incidents and near-misses that happen to the Carers during the course of their shift
- to take responsibility for telephone enquiries from the 24 hour advice line, to give professional advice and support and contact the Doctor on call as required
- to develop and participate in revision of policies, procedures, protocols, care pathways and documentation
- to provide staff support on a day to day basis, to address staff issues as they arise and to promote methods, such as clinical supervision, for personal and professional development

- to support learners undertaking appropriate health and social care courses, offering mentorship as appropriate
- undertake holistic assessments of patients and families that have been referred to the team, this can be in the home, clinic setting or virtually
- to provide nursing care that is patient and family centred, acknowledging the needs of individuals and ensuring high standards of care are provided
- to develop skills in other community related departments within Dorothy House Hospice Care which are relevant to community services which improve the patient and family experience of DH
