Responsibility

  • coordinate and manage care packages for clients in supported living and complex care environments
  • liaise with clients, families, and healthcare professionals to assess care needs and develop tailored care plans
  • oversee the recruitment, training, and scheduling of care staff, ensuring adequate coverage for all client needs
  • maintain accurate and up-to-date care documentation, ensuring compliance with CQC regulations
  • conduct risk assessments and ensure safeguarding practices are in place for all service users
  • monitor and review care packages regularly to ensure they meet the evolving needs of clients
  • build strong relationships with external stakeholders such as local authorities, healthcare providers, and social workers
  • respond to client and family queries, concerns, and complaints, ensuring timely and effective resolutions

Requirements

  • significant experience as a Care Coordinator or similar role within the supported living and complex care sectors
  • extensive knowledge of CQC regulations and compliance in care settings
  • strong organizational and time-management skills, with the ability to manage multiple client cases and staff schedules effectively
  • excellent communication and interpersonal skills, with the ability to liaise with clients, families, and healthcare professionals
  • strong IT skills, including the use of care management software for scheduling, documentation, and reporting
  • A proactive and problem-solving mindset, with a commitment to providing high-quality, person-centered care
  • providing care: 3 years
  • experience in coordinating care for individuals with complex care needs, including those with learning disabilities, physical disabilities, or mental health challenges
