coordinate and manage care packages for clients in supported living and complex care environments
liaise with clients, families, and healthcare professionals to assess care needs and develop tailored care plans
oversee the recruitment, training, and scheduling of care staff, ensuring adequate coverage for all client needs
maintain accurate and up-to-date care documentation, ensuring compliance with CQC regulations
conduct risk assessments and ensure safeguarding practices are in place for all service users
monitor and review care packages regularly to ensure they meet the evolving needs of clients
build strong relationships with external stakeholders such as local authorities, healthcare providers, and social workers
respond to client and family queries, concerns, and complaints, ensuring timely and effective resolutions
Requirements
significant experience as a Care Coordinator or similar role within the supported living and complex care sectors
extensive knowledge of CQC regulations and compliance in care settings
strong organizational and time-management skills, with the ability to manage multiple client cases and staff schedules effectively
excellent communication and interpersonal skills, with the ability to liaise with clients, families, and healthcare professionals
strong IT skills, including the use of care management software for scheduling, documentation, and reporting
A proactive and problem-solving mindset, with a commitment to providing high-quality, person-centered care
providing care: 3 years
experience in coordinating care for individuals with complex care needs, including those with learning disabilities, physical disabilities, or mental health challenges
