  • undertake the duties of the Support Worker as required in order to maintain personal skills in delivering reablement /support
  • to be responsible for overseeing the day to day operation of the services and for all aspects of staff support, including team performance, staff development and attendance management, work schedules and training
  • participate in the Out of Hours Duty System to support staff and service outside of office hours
  • liaise with Health and social care partners to process referrals in a timely manner and facilitate safe interventions of support whether from Hospital or Home starts
