Responsibility

  • patient Outreach o Following up non-attenders for screening, FITs, appointments relating to cancer pathway etc
  • o Helping to embed systems within general practice for cancer pathways o Assisting with 2ww referral pathway; ensuring all required investigations are done prior to referral · Teledermatology support · Project support for delivery of early cancer PCN DES requirements · Supporting practices with prompting GPs for QOF CANOO4 and deliver QOF CAN005 · Embed coding into General Practice systems · Support delivery of education across General Practice · Facilitate regular group meetings · Accessing and presenting existing data · Campaign Engagement across practices/local community · Outreach for hard-to-reach groups of patients · Holistically bring together all of a person’s identified care and support needs, and explore options to support these needs
