Responsibility

  • lead and manage outreach initiatives to promote the services of Community Living Limited in the Bromley area
  • provide hands-on support to the client, ensuring their physical, emotional, and social needs are met
  • supervise, mentor, and provide guidance to the support team, ensuring the highest standards of care are maintained
  • develop, implement, and review individualised care plans in collaboration with the client, their family, and other professionals
  • coordinate and manage shift schedules, ensuring adequate staffing at all times
  • handle escalated issues or concerns and liaise with senior management as necessary
  • conduct regular team meetings and training sessions to enhance professional development and ensure best practices
  • monitor and report on the effectiveness of outreach initiatives, making recommendations for improvements

Requirements

  • NVQ Level 4 in Health and Social Care or equivalent
  • strong leadership, communication, and interpersonal skills
  • proven experience in team management and hands-on care provision
  • ability to work flexibly, including evenings, weekends, and overnight shifts
  • A valid driving licence and access to a vehicle is preferred
  • an enhanced DBS check will be required for this role
  • A minimum of 5 years of experience in outreach or community engagement roles, with at least 2 years in a managerial or supervisory capacity
