- in order for us to provide ‘Outstanding Home Care Services’, We are currently looking for an experienced Care Coordinators who is compassionate, motivated and committed individual
- if you want to join a dynamic team making a positive difference to the lives of service users, then this could be the role for you
- reporting to the Branch Manager on a weekly basis on activity levels and outcomes for service users
- liaising with external organisations in respect of developing a total package of care for a service user
- conducting quality and spot checks
- providing written reports of all complaints, accidents, incidents and untoward events in accordance with written policies
