Responsibility

  • we are looking for a compassionate and dedicated Care Manager to lead our palliative care team
  • develop comprehensive care plans for patients in palliative care
  • coordinate care services and ensure the delivery of personalized, holistic care
  • provide support and guidance to patients and families throughout the palliative care journey
  • collaborate with healthcare professionals to address the physical, emotional, and spiritual needs of patients
  • monitor the effectiveness of care plans and make adjustments as needed
  • ensure compliance with palliative care standards and ethical guidelines
  • the Care Manager will be responsible for overseeing the delivery of palliative care services, developing care plans, and providing support to patients and their loved ones

Requirements

  • degree in Health & Social Care or a related field
  • experience in palliative care or end-of-life care settings
  • strong interpersonal and communication skills
  • ability to work collaboratively with a multidisciplinary team
  • knowledge of palliative care principles and best practices
