Working hours

Responsibility
- work with the Care Manager to develop and implement care plans for clients
- coordinate and schedule care services for clients in their own homes
- assign tasks and delegate responsibilities to care assistants
- ensure compliance with all policies and procedures related to client care and safety
- communicate regularly with clients and their families to assess their needs and preferences
- monitor and report on client progress to the Care Manager
- participate in the recruitment and training of new care assistants
- maintain accurate records of client care and progress
Requirements
- minimum of 1 year of experience in a care coordination role, preferably in domiciliary care
- knowledge of health and safety regulations related to domiciliary care
- excellent communication and interpersonal skills
- strong organizational and time management skills
- ability to work independently and as part of a team
- must have a valid driver’s license and access to reliable transportation
- must be able to work flexible hours, including evenings and weekends
- must be able to pass a background check
