Requirements
- high School diploma required; Bachelor’s degree preferred
- must be registered with Birmingham, West Midlands as an RN. Certificate or equivalent required
- minimum of two years experience in long-term care setting preferred
- nursing: 1 year
Responsibility
- provides skilled nursing and geriatric care
- provides clinical, social, and communication support for patients
- provides transportation, medication reminders, and direct patient care
- records all care activities in the Care Manager’s software system
- reviews discharge plans and related information with patients at time of discharge
- assists patients with self-administration of medications, provides treatment referrals, and assists in the provision of medication and supplies as needed
- performs other duties as assigned by the Registered Nurse Director or Supervisor
- assists in the development of individualized plans of care for patients based on the patient’s medical history and history of past treatment
