Responsibility
- to organise the care team whilst considering the changing needs of service users
- to carry out Individual Needs Assessments for new and potential clients
- this job description reflects the immediate requirements and responsibilities of the post
- to compile weekly care schedules ensuring that all service user calls are covered with suitably trained and experienced staff
- to react to changing circumstances such as staff absence and ensure care calls are still covered
- to carry out introductory visits every time a Care & Support Worker visits a client for the first time
- substantial changes to the job description will be carried out in consultation with the post holder
- to maintain electronic records and hard copy information such as care plans, risk assessments & service user review records
Requirements
- experience / Qualification in Health & Social care
- enhanced DBS disclosure
- driving license and access to a vehicle
- please note this role will involve evenings & weekends to provide on-call support out of office hours
- providing care: 1 year
