Responsibility
- job Responsibilities Help the Practice develop and deliver specialist primary nurse led services including chronic disease management, specifically Diabetes
- maintain disease registers in liaison with administrative staff
- main Responsibilities The Practice Nurse will work as part of the primary healthcare team, providing personal medical services to the patients of the Practice
- initiate the role of diagnosis for patients suspected to have a chronic disease, referring to other clinicians as appropriate Manage programs of care for patients with chronic diseases by planning, providing, and evaluating care under agreed guidelines and patient group directions
- they may also have access to information relating to the Practice as a business organisation
- they do so in confidence and have the right to expect that staff will respect their privacy and act appropriately
- confidentiality In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carer, Practice staff and other healthcare workers
