Requirements
- completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council
- minimum of 2 years post-graduate experience in pharmacy
- member of the Royal Pharmaceutical Society
- member of or working towards RPS Faculty membership
- hold or be working towards a prescribing qualification
- hold or be working towards a postgraduate pharmacy qualification
- clinical pharmacy: 2 years
Responsibility
- providing clinical expertise and being an educational resource for colleagues in relation to medicines
- liaising with and developing relationships with a multidisciplinary team
- managing the process of medicines reconciliation on admittance or following discharge from hospital
- managing, auditing, and reviewing prescription policies to improve the quality, safety, and cost-effectiveness of prescribing
- reducing wasteful polypharmacy and inappropriate and wasteful prescribing of medicines
- proactively seeking to reduce medicine-related hospital admissions and readmissions
- handling patients and healthcare professionals’ prescription queries
- researching and identifying patients that require medication reviews
