Requirements

  • completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council
  • minimum of 2 years post-graduate experience in pharmacy
  • member of the Royal Pharmaceutical Society
  • member of or working towards RPS Faculty membership
  • hold or be working towards a prescribing qualification
  • hold or be working towards a postgraduate pharmacy qualification
  • clinical pharmacy: 2 years

Responsibility

  • providing clinical expertise and being an educational resource for colleagues in relation to medicines
  • liaising with and developing relationships with a multidisciplinary team
  • managing the process of medicines reconciliation on admittance or following discharge from hospital
  • managing, auditing, and reviewing prescription policies to improve the quality, safety, and cost-effectiveness of prescribing
  • reducing wasteful polypharmacy and inappropriate and wasteful prescribing of medicines
  • proactively seeking to reduce medicine-related hospital admissions and readmissions
  • handling patients and healthcare professionals’ prescription queries
  • researching and identifying patients that require medication reviews
